Be an advocate in our community and apply for awards to support your efforts in advancing heart health.
Thanks to the generous contribution from Dr. Norman Campbell, this award aims to acknowledge and celebrate the efforts of those making a difference in Southern Alberta by preventing cardiovascular diseases through community-based health promotion or primary prevention initiatives. The recipient(s) of this award will be granted an honorarium of up to $2,400 which can be used to support:
- A 12-month cardiovascular disease prevention community health promotion project or primary prevention programming and/or
- Individual or group travel to Calgary to receive the award.
Application Process:
Electronic submissions should be made via the online form. Applications should include a 1 to 2-page summary of their project/program and rationale for the community recognition award which will be reviewed and scored based on the following selection criteria.
The following criteria will be considered during the selection process:
- Evidence of applicant’s commitment to promoting cardiovascular health through community programming or health promotion
- Significance and quality of the application, program, or proposed project
- Application, project or program fit with the mandate of the Libin Cardiovascular Institute
